I believe it is defined as the difference between (Max-min) and 0. That is, the peak-to-peak voltage compared to 0 (or the noise floor, once actually implemented).

Right. Your zero point is ultimately arbitrary. If you set your zero point in the middle, you've got 32,768 positive values and 32,768 negative values for a total of 65,536. If you set it at the top you've got 65,536 negative values. If you set it at the bottom you've got 65,536 positive values. And of course you can stick it anywhere in between.

Dynamic range simply denotes the greatest possible change. And with 16 bits the greatest possible change is from 0 to 65,535, or 65,536 possibilities. Though dither allows information to be encoded below 1 LSB so the effective dynamic range can be greater than 96dB.
