I've seen it

I've seen that diagram before. Exclusive mode bypasses the mixing function. However, the diagram doesn't show enough detail about the path the audio data stream follows to know what other functions are in-line or bypassed. For example, what about room correction (and other DSP)?

I notice that KingDoughnut is having some problems using WASAPI. (you?)
The first suspect should be an output buffer running dry. Vista moves a lot of stuff from kernel mode to user mode. That increases vulnerabilty to latency problems and buffer underruns. Try cranking up the buffer size.
