No, that's not it. The "window" on the resistor values isn't so tight that moderate variations will destroy the IC. Did you really tear it all out after only one failure? Maybe it was just infant mortality, or maybe a cheap Chinese IC? I'll also say again that this IC needs a protection diode in the circuit. Tubes do odd things sometimes, and you have to protect SS circuitry from all eventualities if you want it to last.
