These are real and valid concerns. Of the various direct coupled topologies, the Monkey-on-a-stick does provide some protection and negative bias voltage to the grid of the output tube even if the input tube fails/loses connection. Of course then, this topology needs a very high quality shielded choke to load the input tube, which isn't inexpensive....
