If the getter at the top of an output tube is almost clear, while the neighboring tubes have getters that are a solid black and shiny, can one safely assume that the tube with the light getter is "bad" or at least marginal?

and it was originally the same as your other tubes are now I would guess that it is gassy and will die far sooner than the others. It may sound fine for a while yet, but its probably on its way out.
Not necessarily! I had a quad of 7868's and one had the top getter almost gone. The side getter was still all there and it tested and sounded great.

For the most part it should be approaching the end of life. But, I have been surprised before on some that still measure fairly strong in that condition.
