Apart from the (hopefully only) early years of no money, poor diet and spending long hours in an uncomfortable van, the life of a jobbing musician can wear the body down.
Not even considering the opportunities for chemical binging and long mornings to recover from hangovers, even just standing (at least how it used to be) on a stage in front of hundreds of smokers is a considerable health risk from passive smoking.
Bad backs from humping gear up and down narrow staircases, and the continual worry of paying bills with an irregular income.
Its no wonder there seem to be a lot of musicians who die early.
