Read contextually, the patient thinks he must be all right because he feels good. That's theory, too.The doctor for some reason is harping on how the patient must feel, which is a methodological error, but his casual observation indicates something is wrong. The doctor really needs to find out what is wrong, and that would require tests--granted, how the patient feels is relevant information but it hardly settles the matter.
