Occasionally, one of mine stays on all the time. At that point, I put 6SN7s in until is doesn't light up. What is wrong, if anything?

If the DC Offset led stays on, it usually means a 6SN7 that has sections that are far enough out of match that the servo cannot correct it. So replacement is what you do.This does not mean that the tube is bad, just that it needs one with better matched sections, although usually it will work with most tubes that you find.
