The 7591A has a higher plate voltage capability for those amps that may run at 450V B+ or higher. You can use the 7591A in place of a 7591 but you shouldn't use a 7591 in place of a 7591A.
I never compared a 7591 to a 7591A so I can't answer the question if they sound different. Wouldn't be a fair test unless both were from the same manufacturer and they all had the same emission. NOS tubes can be pretty much all other the place. You may need to have 8 to get a very closely matched quad.
The labeled brand does not always tell who actually made the tube so unless you know your tube construction it's kind of hard to tell.
Different brands do sound different. Right now I prefer tubes labeled GE in my Mac 225. Others like the old RCA's. Just personal taste.
As for matching with a tube tester, that's not too accurate unless you have a top notch tube tester. Even a good reading on a lesser tester doesn't mean that the tube is really good when subjected to real operating voltages. I have a Hickock 800A that's regared as being a good tube tester but it's only a rough guide in my experience.
