Systems are too complex, even on a macro scale. A failed controller for an automobile's LED tail light can result in a no start/no crank condition.
And there are plenty of examples of human beings doing things to prevent catastrophe and causing one instead. The SS Eastland, a Lake Michigan excursion ship, capsized while loading and 840 people died. The cause? Lifeboats added after the Titanic disaster, and the additional bracing needed to carry them.
