Channel Master 6601 - Fixed Bias or Cathode Bias? Also SAM's #?

Howdy,
Can anyone tell me if the Channel Master 6601 is a fixed, fixed-adjustable or Cathode Bias unit? (4 x EL84 5 x 12AX7 and a GZ34/5AR4 rectifier)
Also, does anyone know the Sam's # for this one?
