Yes, I had tested All the tubes and they all tested fine.... But as you know you test them in a more static state than in actual use. After a few hours use the tube 'changes' due to heat and that is where I suspect the tube failed.

So, I have already thrown out the entire quad of power tubes and replaced them with a new Quad of tubes. That was a while ago and the amp has been running fine ever since.
