"Sleeping sickness" seems counter-indicated, as the cure is to leave the tube on the tester with the filament fired up at normal voltage, but with the test button not pressed (thereby not applying plate voltage and not drawing current). In fact, it would seem that that method would encourage sleeping sickness, but you did say many hours were required, and I've just done it for 1 - 4 hours.
The third explanation, cathode forming, seems most likely of the three, but it's unclear why one tube would be affected and the other tube not of a production pair. I have read about rejuvenating really old tubes (before they used oxide cathode coatings) by running the heater over voltage for about 30 minutes to cause the emissive material to migrate to the surface.
