Unless you can lift the ground on the source side you need to use a DC to DC Converter to get the isolation. Lots of these from the usual suspects like Farnell and Radiospares. Use a BNX002 filter block (or similar) followed by some good tantalum caps plus 100nF ceramics on the output to get rid of switching noise from the converter. Check costs - which will depend upon how much current you need to draw from it. If you need heavy 5V current it maybe cheaper to drop in another power transformer/rectifier/filter.
