I note that Toole has changed his mind somewhat between the moment he wrote his AES paper and the moment he wrote his book. In his paper he concludes that reflections are not problems whereas in his book he recommends treating front and rear reflections, this without quoting any further literature (as compared to his paper) relevant for this specific point and without giving any reasoning different from that given in his paper (before you ask, by now I have the book). He's perfectly entitled to change his mind, I'm just wondering why and for what reasons
As for spreading of image: all research cited by Toole uses a single lateral reflection. Toole says that the effects of a single lateral reflection on size and position of the auditory image are relevant to live performances and, in stereo reproduction, they relate to sounds that originate in only one loudspeaker.
While sound originating from only one loudspeaker certainly does exist, and in these cases an short-lasting image spread might be observed, depening of how strong the reflection is with respect to the direct sound, I guess that in most cases sounds originate in both loudspeakers, with subsquent reflections from both sides. This case has not been investigated so one simply doesn't know whether or not there is spread or shift of image. Intuitively I would say that these two reflections generate a phantom image of their own, centred midway between the two loudspeakers.
