The C-B junction is reverse biased so the there is a depletion region at the junction preventing all current flow except for Icbo which I believe is due to impurities forming recombination centers. Forward biasing the B-E junction injects carriers into the base allowing the collector current to flow through it as a Fn (beta) of the injected current.
