Do you use an iPhone and is it on Wifi when you are at home ?If so, go to the Settings Icon > Wifi. It will display the SSID that you are presently connected to, which is more than likely to be your home Wifi.
The SSID is essentially a name that identifies a wireless network. It can be a random name set up by your ISP or a familiar name that you choose. My Dad's Wifi SSID is USMC as he is a retired Marine. Of course, you will need to know your SSID and the password to log devices onto your Wifi network.... assuming a password was assigned when your Wifi was first set up.
Otherwise, there are easy ways to 'see' nearby Wifi networks on your Mac in System Preferences > Network > Wifi. You might 'see' an SSID that you recognize.
