I use the systemC134 from Logic supply, its pretty small and fanless. It does have a hard disk and optical drive. Without the fan it runs warm but not hot, just make sure you don't put anything on the top that would block the ventilation holes!For one significantly smaller try the Damn Small Machine, brought to you by the Damn Small Linux folks. No hard disk or optical drive (its not big enough for that), but can be had with various amounts of flash disk (or none, you can run it direct from a USB flash drive). Of course you can get it with DSL or not and load your own OS. Since it does not have a hard disk you can either use it as a local player for a separate server or plug in USB or NAS drives. I wouldn't use it as a gaming machine though.
