Yes, IEM's can sound very very good. Better than many "real headphones" to use your terms. Some people use them as a main headphone source. Are they better than ALL over the ear headphones? I don't think anyone would make that claim.
The down side is that they sit inside the ear canal which some people find uncomfortable and they typically block exterior sound (so you would not like them if you need to hear the environment around you).
