Hi Steve, I am not sure about the configuration as I used mine on a driver tube for the LadyDay. You can calculate the wattage by measuring the voltage drop across the CCS and multiplying by the current you set. At 3 watts you can get by with a clip on heatsink. Regarding the board, throw it away. It gave me more headaches than it is worth. Solder the necessary resistors right to the terminals as per the schematic (that I hope you have). The board is mis-labeled making it more confusing than if you just solder right to the chip. The specs for the chip can be found on the net. The data sheets will show you the terminal connections and all other vital info. If you do mount the chip to a heatsink using a mica insulator be sure not to over torque the mounting screw . Check with your VOM to be sure that you have not shorted the body of the chip to the heatsink.
