My gut feeling is that it is a Bussman product-
there appear to be some markings on the top of the device; however I could not read them-
If you can identify the fuse series/type, you can search that way -
the Bussman site has a lot of details-
Based on what you have provided, here is the cut sheet for the fuse-
you should be able to find the holder...

The text that is visible in the pic states the following: FOR CSA ONLY, USE ONLY WITH A 250V FUSE. It is repeated in French: EMPLOYER UNIQUEMENT AVEC UN FUSIBLE DE 250V.
I went to DigiKey and tried looking up Buss fuse holders but there are hundreds of variations, best I could tell. I was hoping there was some nomenclature specific to this type of holder.
When I Google the French version of the text I get close. See the link. But the amp does not have a switch included in the assembly, just the IEC connector, and the fuse holder with a single fuse socket inside. And now I'm wondering if the removable piece can even be purchased separately. I probably just need to clean it up best I can.
CSA is an acronym for Canadian Standards Association.
