1. Are you using the latest version of sacd_extract?
2. Have you tried ISO2DSF for the conversion of your ISO?
3. Consider making the shift from iso2dsd to the new SACDExtractGUI and its improved version of sacd_extract (not compatible with iso2dsd).

Thanks for the suggestions everyone! I tried a few version of sacd_extract including SACDExtractGUI per the suggestions. In the end, I believe the issue was the path/filename length. I tried extracting to a directory close to the root to shorten the total path/name length and it worked.

I've ripped ~170 SACDs without a hitch using it.
