If you want to wire for balanced operation, you will need four wires (not counting the shield). If you don't desire balanced operation, there isn't a real need for the 4th wire.

how it is meant to be used. Sometimes they mix types i.e., two high purity copper and then something of lesser quality. It all depends upon how the wire was meant to be used and for what.

The quality of the "ground" return path is equally as important as the hot.
My comment above was referring to what type of wire was being used. Sometimes manufactures do not use the same quality for ground return as in my Ultrasone Ed. 9.
