I don't think that's the 9595 horn, I think it's the 9597 or whatever the other one in that series is. It's too tall for a 9595. The one you have is designed for more narrow throw than the 9595, it was usually aimed higher and meant to reach the balcony, with the 9595 covering the orchestra (in a typical theater).

The diaphragm will be hard to come by, expect at least $250 for one if you can find one. Rear cover will be hard to find too, they were also phenolic/bakelite and somewhat fragile/brittle.

The 9595 and it's brother are nice horns but you need a big space to really let them air out the sound. Most of the time they are too much for a home environment. I had a pair of 9595s for a while.

The driver is a nice unit, I use a pair of those still - really great for vocals.
