Pickup Arm Design by Stevenson

I´ve downloaded the paper on Pickup Arm design by Stevenson from this "asyllum" but the pdf file have an error. One page is missing...
There´s one page between page 7 and 8 (page 316 on part 2 of the original work) but it only blinks on the screen and quickly hides. Is not readable or printable.
Can someone fix it or inform another place I can download it?
Also, where can I find the Lofgren work on the same subject but translated to english?
