here's the schemo and pinouts for 45/2A3

For the 45 (or 2A3, the same for either one):
pin 1 - filament
pin 2 - plate
pin 3 - grid
pin 4 - filament
(Viewed from below the socket, with the two FATTER pins at the bottom, it looks like this:

1 4
1 and 4, the filament pins, are the fat ones.
The driver pinout I will let you look up yourself 'cause I can't remember it at the moment and my wife is asleep so I can't turn on the light to see where my tube book is.
