I just remembered a time where I had a cold solder joint at a jack that was giving me grief. In your case it could be a solder joint for your output jacks. Especially if they share a common ground.
I also had an issue, but with input jacks, where the ground was formed by the jack being fastened to the chassis. The chassis and jack metal had to be cleaned to fix the problem.
