Happens often enough

I have a double album that is completely jumbled up and quite a challenge to get to the side I want. The disc one and two are mixed up, and on one, the sides A & B are switched, but on the other, the sides (but not the disc) are at least in order. I could never get things right until I relabelled the disc. By the way, the album is a collection of Leonard Cohen songs done by other artists named "I'm Your Fan."

I once had a record that was pressed with two side 2s. I also have a classical record by Argo where the speed is completely, and radically off -- it is not 33 1/3 or 45 or any recognizable speed. How in the world did this get mastered, manufactured and released without anyone listening to the result?
