You can buy choke from partsexpress.com for about $36 plus shipping. That's where i got the choke.

for the connection, i use a wire (14 gauge, 3 prongs). And i use wire nut to connect them. I'm sure someone with more expertise than i do can give you better and detail connection.

I plug the choke in an outlet which is also in the same line of the outlet where i plug in my power conditioner.
