It can be intermittent joints, bad caps or tubes, or far worse - stability problems where on some loads, signal frequencies and amplitudes amp produces burst of (HF) oscillation. Usually, this is not obvious with single frequency and dummy load, but with complex (music) signal HF oscillation may be "triggered" and sound is distorted...
