DC,It looks like you have eliminated the possibility of a bad tube causing this; have you also ruled out that the problem may be in the preamp? A subsonic disturbance could cause this problem- does the amp behave this way with the input disconnected? How about when connected to the other speaker (never run a tube amp with no load attached).
Beyond this, I would have to go with a leaky capacitor already suggested. It would be hard to track this down without test equipment, but working backwards through the circuit, suspect any coupling cap in the signal path. This may take some trial and error.
