A cathode follower takes its output from the cathode. As such, it has 100% negative feedback and therefore the gain is slightly less than one. Because of the feedback, it has very low output impedence. It is therefore used as a buffer stage to provide a low impedence output.
Because it has 100% feedback, it should have very low distortion. However, many enthusiasts shun it because of a reputation for poor sound quality. My EAR 834P uses a CF at the output and it doesn't sound all that objectionable. There are ways to improve the performance, the most important of which is the use of a constant current sink as cathode load.
