I have only seen one of these too. You are correct about the date code. These have flat plates with dual mica "snubbers" that run the full length of tube. In my opinion, probably made for military (mine came out of a piece of Army eqpmt..)by RCA. I sold it for $75, didn't think I'd ever come up with a quad for my amps :(
