Do you use an iPhone and is it on Wifi when you are at home ? If so, go to the Settings Icon > Wifi. It will display the SSID that you are presently connected to, which is more than likely to be your home Wifi.
Otherwise, there are easy ways to 'see' nearby Wifi networks on your Mac in System Preferences > Network > Wifi. You might 'see' an SSID that you recognize.
