It does sound like she may be suffering from a type of depressive episode.The fact that smoking ( tobacco ?) has returned indicates she is not dealing with stress very well.She has lost a job that may or may not have been related to her presumed decline.Was job performance an issue in the past? If she is in fact depressed now or in the past she may have a case to keep her job and should contact her EAP while she still can.
You did see some issues a year or so ago and that your wife did not want to address them. That said she in all likelihood does not recognize that there is a problem. She does need to find someone that she trusts to talk with especially is you have any concerns grater than her well being meaning her safety.Hard to really tell in this format.
I wish you the best and if you need to ,see someone yourself.This has to be an extraordinarily difficult period for you.
