Ground wire inside DRD 45

I recently completed my DRD45 amplifiers and each has a separate issue. On one amplifier, after the first hour of connection to my system, I heard a loud pop from one of my speakers. I looked at the 45 tubes on the amplifiers and one was glowing normally (a nice golden glow) while the one connected to the speaker that "popped" had a swirling blue glow inside the tube. I immediately turned off this amplifier and soon after replaced the tube. The new tube has been running for about ten hours with no problem. Does this sound like just had a bad tube, or is there something I should check within the amplifier?

The other amplifier has had a loud low frequency hum since I first connected it in my system. I was trying to figure out the cause, and while checking things, I removed the ground wire from the IEC plug connector leading to a bolt on the chassis (the other ground wire connected to the bolt is still connected) and the hum disappeared. Should I just keep this ground wire disconnected? or is this dangerous, and should I search for a different cause for the hum?
