I have experienced these problems myself from irresponsible carriers. It appears the item dropped off a conveyor or simply thrown.

Strange story- I sent a 70lb electronics item to FL. It was damaged with a bent chassis, twisted transformer & other details. I got the claim paid. A crushed lizzard was under the box & pictures were taken of this event. The box was obviously thrown off the truck as the lizzards are very fast moving creatures.
