You need a PS bracket and the terminal boards to put the amp back together. I don't have a bracket nor do I know of anyone who can supply a dimensional drawing to have one made. The bracket also serves to reinforce the chassis top deck to prevent the 15 pound power trafo causing it to sag.
IIRC you can buy "hole shrinkers" to allow use of a 9 pin socket where the 5691s are now. Why were they installed instead of the 12BY7As? Well, everybody knows that pentodes are no good, right? :-)
Maybe it's my old eyes but I don't see any toggles on the top deck. I DO see replacement pots in the holes where the AC balance pots were originally.
I would try to save those old yellow Rel-Caps, i still think they are a fine cap. They are probably polypropylene.
You've got a lot of work ahead of you, but when it's all done you'll be very glad you did it!
