They rely on the insulation of the power transformer. This is OK most of the time, but I wouldn't trust my life to it. Unless the transformer is extremely low capacitance (medical grade), it can probably pass sufficient current to stop the heart if your skin is wet or broken.

No, but you need to be careful when working with equipment that has an earthed chassis. The problem isn't the earthed piece per se, but in the presence of equipment that isn't earthed, it presents a path to ground. If you touch an unearthed piece at the same time as an earthed one (such as when plugging in a phono plug between two such pieces), the body will complete the current path.
