It does sound like a ground loop with the preamp.
You could plug a cell phone in instead of the preamp to see if it still hums or not.
I have isolated RCAs which are grounded only at the local ground start for the driver tube stage.
Most times IME it does have something to do with the way the RCA wire is configured.
Sounds like you are on to something. I'm confident you will get a scheme when you do your build.
