I assume conventional white thermal paste like Dow 340, not a silver filled paste for CPUs. Those pastes are no good for HV. It would be hard to imagine a Sil-Pad or mica leaking. If you have a burr, you'll have a direct short circuit. Never use grease with a Sil-Pad. I'd be very suspicious of the little top-hat washer that goes in the TO-220 hole. Those can be made of nylon or other stuff that absorbs water. Find a better/different washer or try using a Delrin screw. Also make sure you don't have some missed leakage path from the center lead of the package and not the tab at all.
