The screw, if conductive, can't touch one of, the heatsink or to220 tab. So...for a tapped hole..you want a bushing that the screw goes thru....that fits through the hole in the to220 tab....and extends into an undercut in the heatsink. This centers the to220 device so it can't shift and have the screw touch the to220 case. Also insures the bushing is short enough so when the screw is tight the to220 device is in firm contact with heat sink.
With typical through hole mounting this bushing extends through the hole in the heatsink from the backside. It centers the screw in the through hole so it can never touch the heatsink.
Probably not doing a good job explaining but a goggle search will reveal why tapped heatsink and high voltage aren't normally used.
