I think you and I were trained in a similar era. I don't remember precisely what the terminology was back then, but from the materials I see on the Web today, it looks like your four kinds are now called:
a) series-shunt feedback (a voltage amp using voltage-series feedback)
b) series-series feedback (a transconductance amp using current-series feedback)
c) shunt-series feedback (a current amp using current-shunt feedback)
d) shunt-shunt feedback (a transresistance amp with voltage-shunt feedback)
The first term of the pair refers to the type of input/summing (series = voltage, shunt = current) and the second term refers to the type of output/sampling (series = current, shunt = voltage).
