AD797.
The latest generation opamps, like LME49710, are superior to the AD797 in all but one regard (they are also cheaper, I believe). But they have higher input noise. They were designed for digital formats and not MC carts. You might want to try the LME49710 first.
The ancient and expensive LT1028 is the lowest noise opamp I know of. But I prefer the AD797.
