Since it's only happening on that particular tape, it's quite possible that the tape has been exposed to a magnetic field which has affected the recorded signal. Play the tape from its original reel, and watch to see if the "fade out" is related to the rotation of the supply reel (e.g., fade-out once per revolution).

Proximity to power cords and transformers will have this effect.

Another possibility is that the tape has been exposed to uneven heat, e.g., part of the reel was in sunlight while the rest of the reel was not.
