Updated cartridge set-up links

Rod,

I have, long overdue, updated the cartridge set-up links, see the source code for the list pasted below.

It turned out that most of the external links hade become dead, so I found the new locations of the files. I also added a few new entries (I hope that's okay) and deleted a couple. Also changed the sequence a little bit.

While it was an honour to see my name in the heading, I wouldn't mind to have it deleted, but do as you like. It looks kind of mysterious, perhaps.
