To add to this, when you turn the amp on, if the first tube isn't a directly heated tube, you'll have exactly this condition. (A 2A3 is basically "instant on")
A very, very small percentage of new production tubes will arc under these conditions. This can be avoided with a very slow starting rectifier tube, with monkey-feed topology, or with more advanced loading techniques that control the driver plate voltage when no current is being drawn.
