RE: Grounding isolation

The chassis, frames of your iron, and IEC ground should all tie to the chassis at one point, generally as close to the AC power entry as possible. If there's an issue in a transformer, or somehow part of your circuit tries to make the chassis hot, your fuse will pop and you'll know something is wrong.

As far as how you connect the audio ground to the safety ground, YMMV. Generally I go for the quietest configuration, which tends to be connecting the audio ground to the chassis at roughly the midpoint of the first gain stage.
