Page 8 of the manual shows how unbalanced cables (1/4" or XLR) should be wired. Are you using a cable with a mono 1/4" plug where the "ring" is grounded (like the illustration)? Some plugs have an insulator instead of the ring, or a stereo plug might have the ring unconnected.

Is the ground jumper strap on the terminal block (linking the input ground with chassis ground) still there?
