Latest Winamp, with DirectX, FFDSHOW, VID4WA 2.7a (not 2.6) plugin, Adapt-X DSP plugin with SourceForge Convolver component, in_aif plugin, in_au plugin, Shibatch mpg123 input plugin, ASIO4ALL, out_asio(dll) plugin v0.67 (be sure to use this exact version), out_fastwave (output for external USB DAC).

The convolver FIR I use can be downloaded from here. I use the 32-bit floating version of this FIR function, with the attenuation set to -6.0 dB.
